To receive your PACE or FL continuing education certificate, you must complete the program evaluation before the deadline (within four weeks of attending the conference).

1. Go to Survey Monkey to complete your workshop evaluation.

2. Follow the instructions and click “Next” to begin.

3. Complete the evaluation. Once you click “Done” your responses will be submitted.

Please Note: You will receive your certificate via email so make sure to type your e-mail address correctly.

4. The evaluation will close 4 weeks after the event date. Your CEU certificate will be emailed approximately 5-6 weeks following the workshop. Once the evaluation closes you WILL NOT be able to request CEUs.
